Preparing For Your First Visit at Hawaii Kidney Specialists
Please take the time to fill out our new patient packet in its entirety, and bring the completed packet with you to your first visit. You can also fill out the packet online and email it to our office directly. If these options are not possible, please arrive at your appointment at least fifteen minutes early to fill out the necessary paperwork.
The new patient packet consists of the following:
- New Patient Registration Form
- Medication List
- Medical Records Release Form
- Financial Policy
Please bring to your first appointment current insurance cards, valid photo identification, and copayment.
Your first visit with the practice will consist of a complete physical examination and comprehensive evaluation including:
- History of your current problem
- Medical and social history
- Family medical history
- Laboratory and Radiology test results
- Current medications
To determine how your kidneys are functioning or to assist in determining a diagnosis, additional testing may be required. These tests may include:
- Blood testing
- Renal Ultrasound or CT scan
- Renal Biopsy
- Urinalysis
Our office requires at least 48 hours advance notice if you are not able to keep your appointment with us.
